These kind of problems are surely now systemic. With all the centralization of meatpacking and all the cross branding - bad meat can and does get very widely distributed. With the volumes of meat going through the plants - infection is inevitable.
The only way to have safe meat is to have a local system - where the animals are raised and then killed in a low stress environment. No amount of inspection, refrigeration, hand washing etc is going to work so long as the design of the system drives problems.
Of course Food Inspection has been driven by the lobbying power of the big guys - nearly all small and local has been shut out by design. Now we also know that the Harper Government want to go one step further and have "industry" do the inspection itself - So Maple Leaf will be in charge of the system!
